ISO: Survey to identify course in-use course materials

Hi all, I’m supporting a faculty member who is looking to conduct an environmental scan within their department of all learning resources that are being used by instructors, not just OER. The goal is to identify gaps in OER use. Would anyone happen to have access to a survey that was used to conduct a similar scan elsewhere?
